Breakdown of 2025 MediShield Life Premium Revisions on Net Premiums Payable by Income Decile, and Quantity of Unsubsidised Policyholders

17 Dr Hamid Razak asked the Coordinating Minister for Social Policies and Minister for Health following the 2025 MediShield Life premium revisions (a) what has been the median net change in annual premiums payable after subsidies and MediSave top-ups for resident households by income decile; and (b) how many policyholders have received no premium subsidy.

Mr Ong Ye Kung: Around 60% of policyholders from lower- and middle-income households as well as Pioneer and Merdeka Generation seniors receive structural premium subsidies. When MediShield Life was enhanced in 2025, the Government provided an additional $4.1 billion in premium support for three years. This comprised $3.4 billion in additional MediSave top-ups and $0.7 billion in additional premium subsidies.

The net change in premiums varies by age, per capita household income and property annual value. Table 1 below shows the median net change in premiums, broken down by the income tiers for means-tested MediShield Life premium subsidies for those in properties with an annual value $21,000 or less. This does not factor in MediSave top-ups. We do not have data by income deciles.

For more than nine in 10 Singaporeans, the net increase in annual premiums over the next three years will be more than offset by the MediSave top-ups.
